English version

Conclusion. Many wheelchair users spend more than a dozen hours a day in the chair. At that intensity, comfort is not merely a question of how it feels but bears directly on health, through pressure injury, pain, postural deformity and fatigue. Comfort is often treated as subjective and left unassessed, yet it rests on measurable physical foundations: pressure distribution at the contact surface; microclimate, meaning interface temperature and humidity; postural support across the body; vibration transmitted to the body; and the physical effort use demands. All of these can be measured, and once measured, assessment and improvement have a basis.

The role of pressure distribution. Pressure at the ischial tuberosities is the principal risk point under prolonged loading. Sacral pressure arises in semi-reclined postures and with sliding. Posterior thigh pressure arises where seat depth is wrong. Foot pressure arises where footplate height is wrong. Back pressure distribution reflects backrest support. And change over time covers how distribution shifts after a period of sitting. Change over time is the examination specific to prolonged sitting: distribution may be good on first sitting and markedly worse hours later as materials compact and posture slides. Measuring only the initial distribution says nothing about prolonged use.

The role of microclimate. Interface temperature and humidity directly affect skin tolerance. Temperature accumulates at the interface over time. Humidity accumulates as perspiration collects. Breathability of cushion and upholstery governs both. Seasonal differences make summer more demanding. The relationship to pressure injury is that moisture and heat reduce the skin's tolerance of pressure. And measurement places temperature and humidity sensors at the interface with continuous logging. The combined effect of microclimate and pressure is the real risk: at the same pressure, tissue tolerates better on a dry cool surface than on a moist warm one.

The role of postural support. Pelvic support underpins the whole posture. Trunk support depends on backrest shape and angle. Upper limb support depends on armrest height and position. Lower limb support depends on footplate height and angle. Head and neck support depends on a headrest where needed. And posture maintenance covers how long good posture is held. Posture maintenance time is a practical measure: many systems produce good posture on first positioning, after which the user gradually slides and leans and posture deteriorates. Assessment should include the state after a period of sitting.

The role of vibration. Vibration arises from the road surface and the drive system. It transmits from wheels through frame and seat to the body. Attenuation comes from tyres, suspension and cushion. Measurement is made at seat and backrest. Long-term exposure relates to back problems. And the method measures seat acceleration while travelling on a specified surface. Long-term vibration exposure is a genuine health factor for people using a chair for many hours daily, and routine testing does not address it.

Assessment methods. Objective measurement covers pressure, temperature and humidity, vibration and postural angles. Prolonged measurement runs continuously for hours to reflect real use. Subjective assessment records the user's ratings. Skin inspection records the response after use. Postural observation records change during use. And functional capacity records change in ability after prolonged sitting. Combining objective and subjective assessment works well: objective data describe the physical state while subjective assessment reflects the user's experience, and where the two diverge, that divergence is itself informative.

Relationship to health risks. Pressure injury results from the combined action of pressure, shear and microclimate. Pain results from poor posture and pressure distribution. Postural deformity results from prolonged poor posture affecting the skeleton, which matters particularly for children and long-term users. Circulatory problems result from compression. Respiratory function is affected by posture. And digestive function is affected by posture. The respiratory and digestive effects are easily overlooked while being genuine health factors for prolonged sitting: excessive recline or flexion affects both.

Directions for improvement. Cushion selection and configuration address pressure distribution and microclimate. Seating system configuration addresses the position and shape of supports. Dimensional fit addresses seat width, depth and backrest height. Angle settings address seat angle, backrest angle and tilt function. Vibration mitigation addresses tyres, suspension and cushion. And posture change functions, such as tilt and recline, facilitate pressure relief. Posture change functions are particularly valuable for prolonged sitting, since periodic repositioning is an effective pressure relief measure and a chair with those functions makes it easier to do.

How we handle it. For comfort assessment we suggest prolonged continuous measurement rather than initial-state measurement alone, since pressure distribution, microclimate and posture all change over time and prolonged sitting is how these products are actually used. For products claiming comfort or pressure relief we suggest measuring pressure distribution and microclimate together, since they act in combination and assessing pressure alone is incomplete.
