English version
Conclusion. When a wheelchair fails a test, the sequence that follows is analysing the cause, defining a remedial plan, implementing it and retesting. Where that sequence runs badly, the schedule stretches repeatedly: inadequate remediation fails a second retest, remediation introduces new problems, an unclear retest scope leaves something out, or sample preparation falls behind. Smooth handling depends on several things done before remediation: understanding the cause, understanding the consequences of the remedial plan, and settling the retest scope.
The first step after failure. Confirm the failure, covering data, conditions and the basis of judgement. Understand the failure phenomenon, covering under what conditions, at what location and in what manner. Request detailed information, including photographs, data traces and a description of the failure location. Judge whether it is an individual sample defect, where one sample failed and others did not. And confirm that test conditions were correct, excluding setup or operational problems. Detailed information matters for cause analysis: knowing only that static loading failed is insufficient, and the location, the load and the manner of failure are all needed.
Cause analysis. Distinguish immediate from root cause: a weld cracking is the immediate cause, and why it cracked is the root. Consider the categories of cause: design in structure, dimensions or material selection; material in substandard properties or batch problems; process in welding, heat treatment or assembly; or an individual sample defect. Verify the hypothesis through inspection or testing where possible. And use the failure location, which usually points to the cause: a stress concentration points to design, a weld points to process, and an internal material defect points to material. Failure location is the important clue: failure at a designed stress concentration points to design, failure at a weld or heat-affected zone points to process, and failure at an apparently ordinary location may point to material.
Considerations for the remedial plan. Address the root cause, since treating symptoms lets the problem return in another form. Leave margin rather than remediating to just meet the requirement, since batch variation will fail a marginal solution again. Assess side effects, since reinforcing one area may transfer load to another and adding thickness may affect assembly or weight. Assess effects on other items, since remediation may affect items that already passed. And assess feasibility, covering manufacturability, cost and whether tooling must change. Leaving margin is the most practical advice: remediation that just passes self-testing will very likely fail retesting, because measurement uncertainty and sample variation both intervene.
Determining the retest scope. Mandatory items include the failed item. Items affected by the remediation must be retested according to what characteristics changed. Exempt items require a justification that they are unaffected. New items may arise where remediation introduces fresh considerations. And the scope should be confirmed with the laboratory, to avoid discovering omissions after retesting. Items affected by remediation deserve careful analysis: reinforcing part of a frame may satisfy strength while increasing weight and shifting the centre of gravity, so stability and weight-related items may need retesting.
Preparing samples. Retest samples must incorporate the remediation, confirmed before dispatch. Quantity follows the retest items, allowing for destructive testing. Retain pre-remediation samples for comparative analysis. Record the differences clearly for explanation. And ensure consistency between retest samples. Confirming that the samples sent are the remediated ones sounds superfluous and has genuinely gone wrong: sample confusion has caused retests to fail again and wasted a whole cycle.
Communication. Communicate promptly when a failure occurs rather than waiting for the complete report. Provide complete information about the failure. Explain the remedial plan, covering what changed and why. Confirm the retest scope explicitly on both sides. And discuss timing, covering how long remediation and retesting will take. Prompt communication shortens the overall schedule noticeably, since the client can begin analysis and preparation before the report issues.
Avoiding repeated cycles. Self-test after remediation, verifying before submitting for retest. Address multiple problems together where several items failed. Leave adequate margin, as above. Analyse side effects to avoid solving one problem and creating another. And prepare samples adequately to avoid interruption. Addressing multiple problems together saves noticeable time and cost, since remediating item by item means a complete cycle each time.
How we handle it. When an item fails we communicate promptly and provide detailed failure information covering location, load, manner and photographs, since those underpin the client's cause analysis. The remedial plan is the client's decision, involving design and production and requiring their assessment of effects on other aspects of the product; we can comment on how the plan might affect test results. We propose a retest scope with our reasoning, and where the client believes items can be exempted we are glad to discuss a justification.
